Practice Triager

About the Role

Clinical Triage and Assessment Conduct same-day telephone, online, and face-to-face triage consultations to assess,

diagnose and manage patient presentations in line with national and local protocols. Review all incoming patient

requests (AccuRx, eConsult, NHS App, and telephone) and prioritise based on clinical urgency, safety, and suitability

for self-care, allied health, or GP review. Provide autonomous assessment and management for a range of acute and minor

illnesses within scope of practice. Prescribe and review medication safely where qualified as an Independent Prescriber

(V300). Identify red flags, safeguarding issues, and deteriorating conditions promptly and escalate appropriately.
