About the Role
Job Overview The Practice Pharmacist will support the safe and effective use of medicines across the practice. The role
involves a mix of clinical work, patient-facing consultations, and medication management, helping to improve patient
care and streamline prescribing processes. Key Responsibilities Carry out medication reviews and provide advice to
patients on their medicines Support safe, cost-effective prescribing in line with current guidelines Respond to
medication queries from patients and the wider clinical team Review repeat prescriptions and manage medication requests
Work with GPs and nurses to optimise treatment plans Monitor and improve prescribing practices across the practice
