About the Role

Key Responsibilities1. Clinical Care and Prescribing Conduct structured medication reviews (SMRs) and follow-up

consultations. Prescribe independently within competence, following local formularies and NICE guidance. Support

management of chronic diseases (e.g., hypertension, diabetes, asthma, COPD, cardiovascular disease). Review repeat

prescriptions for accuracy, safety, and cost-effectiveness. Monitor high-risk medications, titrate doses, and manage

adverse effects. 2. Medicines Optimisation Implement evidence-based medicines management and optimisation strategies.

Undertake medicines reconciliation after hospital discharge or care transitions. Support antimicrobial stewardship and
