Primary Care Paramedic- Deal & Sandwich PCN

About the Role

This role offers a unique opportunity for an experienced Paramedic to make a tangible difference to the health and

wellbeing of people in our community and care settings. Working at the interface of primary and community care, the post

holder will provide high quality holistic care that aligns with the EHCH framework and the broader goals of integrated

care. The Paramedic will play a key role in our Enhanced Health in Care Homes (EHCH) Team, working alongside paramedic

colleagues and care coordinators, as well as the wider PCN team, including social prescribers, clinical pharmacists,

pharmacy technician, mental health practitioners and MDT Care Coordinator. This roles contribution will support
