About the Role
To work as part of a multidisciplinary 0-19 team to identify the health needs of children and young people, their
families and the community and contribute to developing a strategy to address individual identified needs as well as
those needs within the community. To be committed to learning and development. Post holder will be supported to develop
skills and knowledge appropriate for application to the Specialist Community Public Health Nurse course. To develop the
knowledge and skills to assess, plan, implement and evaluate packages of care in order to deliver early interventions to
families in line with our commissioned service. To support families through a range of methods including home visits,
