About the Role

The Healthy Family Teams deliver Universal and targeted health services to the 019-year population in Nottinghamshire.

You will be responsible for supporting the day-to-day management of a defined Caseload, working within a skilled and

mixed team. This is an excellent opportunity for a suitably qualified, experienced nurse with sound communication skills

to develop their role within the community setting. We offer a friendly and supportive working environment with

excellent development opportunities. As part of a Healthy Family Team, you will be responsible for the delivery of
