About the Role

JOB OVERVIEW

The heart failure/IV iron Enhanced Practitioner will work as a skilled, autonomous practitioner as  part of the

multidisciplinary team across primary, secondary and tertiary services to deliver  high  quality specialist care for

patients diagnosed with heart failure throughout their care  pathway. The post-holder will ensure patients are cared for

in the most appropriate setting, provide specialist advice, support and information from the time of diagnosis through

to end of life. They will also lead the management and co-ordination of the intravenous iron for heart failure patients'
