Reablement Enabler (Sc5)

About the Role

We are looking for a compassionate and motivated Reablement Enabler to support people in their own homes to regain

independence and confidence. This is a rewarding role focused on short-term, goal-based support rather than long-term

care.

You will provide practical and personal support tailored to each individual, helping them rebuild everyday skills

following illness, injury, or hospital discharge. Duties may include supporting with personal care, assisting with

household tasks, and encouraging participation in activity or exercise programmes designed by healthcare professionals.
