Reablement Practitioner
About the Role
As a Reablement Practitioner at Hampshire County Council, you will support individuals in regaining independence through a strengths-based approach. Collaborating with a multi-disciplinary team, you will conduct assessments, develop interventions, and foster positive relationships with service users and their support networks. This role is essential for enhancing the quality of life for those in need of reablement services.
Responsibilities
- Triage referrals and complete holistic assessments.
- Plan supportive interventions with a multi-disciplinary team.
- Provide advice using a strengths-based approach.
- Build relationships with service users and healthcare professionals.
- Support safeguarding cases as needed.
Requirements
- Experience in Reablement or Health/Social Care preferred.
- Knowledge of Care Act 2014 and Mental Capacity Act 2005 is a plus.
- Strong communication and teamwork skills.
- Detail-oriented with risk assessment abilities.
- Demonstrates empathy and respect for service users.

Compensation
Salary range is £33,178 - £36,369 per annum, depending on experience.
Location & Schedule
Work location is Fareham Parkway, Fareham with hybrid working options. Current hours are 08:30-17:00 Monday to Thursday and 08:30-16:30 Friday.
