About the Role

Job Purpose To work as an autonomous practitioner, you will undertake comprehensive assessments to identify and meet the

needs for Individuals under the care of Urgent community response team as well as support with their general assessments

and rehabilitation goals. To carry out profession specific, and holistic, assessments, care plans and evaluation as

required. To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme

for adults with a wide variety of complex physical and mental health needs while individuals are having a period of

assessment. To be responsible for continuing the ongoing development of high standards of clinical practice with
