About the Role

To assess, plan, implement and evaluate client care supported through a robust preceptorship or personal and

professional development programme linked to the Trust’s implementation of the Knowledge and Skills

To co-ordinate the individual’s assessment and subsequent care plan as agreed with the multi-disciplinary team.

The post holder will adhere to Trust policies, Data Protection and Health and Safety.

· To participate as an effective member of the MDT and be responsible for the development and facilitation of a patient

centred approach to care: assessing, developing, implementing and evaluating the care needs of patients and their carers
