About the Role
An exciting opportunity has arisen to be part of the Tameside GM SMI Team.
You will participate in the running and ongoing development of an arm of the Physical Health & Treatment Team, focusing
on patients who have been identified as having a Severe Mental Illness (SMI), and supporting them to access a Primary
Care Physical Health Screen, reducing barriers and helping patients who cannot access services directly. You will work
across Primary and Secondary Care Settings, promoting continuity of care and effective communication.
