About the Role
As a registered nurse care coordinator (RNCC) at UAB Medicine youll join a Magnet-designated nursing team to provide the
highest quality of care even for the most complex cases. UAB Medicine serves a diverse patient population allowing RNCCs
the opportunity to develop and expand their skills. The UAB Care Transitions Department includes more than 200
registered nurse care coordinators and social workers who are integral members of the interdisciplinary team. RNCCs
provide care in both the inpatient and ambulatory settings including the Emergency Department with a focus on screening
and assessing patients for any post-discharge needs providing referrals for in-home care and participating in care
