About the Role

We are looking for Registered Adult Nurses to work within a community integrated team as part of a multidisciplinary

team.

Full Time and Part Time hours considered - Please state your preference of hours on your application.

The service provides nursing and therapeutic input to patients within their own homes with the aim of returning them to

maximum independence, by actively promoting self-management on all activities of daily living.

You will support your peers and team leaders by being an effective team member whilst working on your own initiative.
