Job title:Registered Nurse Hospice at Home Reports to:Hospice at Home Lead Hours: Part-time and full-time opportunities

available (job shares considered) Salary: £19.10 per hour Location:Ashfield, with travel across North Nottinghamshire to

meet organisational needs. We offer part time and full-time positions. Applicants should state their preference as part

of the application process. Main Duties Integrated Working and Care Coordination Work in partnership with the Integrated

Hub, Hospice at Home Lead and Care Navigators. Support discharge planning and reduce avoidable hospital admissions.

Communicate effectively with patients, carers and professionals. Attend and contribute to MDT, GSF and Hub meetings.
