About the Role

Job title:Registered Nurse Hospice at Home Reports to:Hospice at Home Lead Hours: 18.37 hours per week Salary: £19.10

per hour Location: Bassetlaw with travel across North Nottinghamshire to meet organisational needs. Main Duties

Integrated Working and Care Coordination Work in partnership with the Integrated Hub, Hospice at Home Lead and Care

Navigators. Support discharge planning and reduce avoidable hospital admissions. Communicate effectively with patients,

carers and professionals. Attend and contribute to MDT, GSF and Hub meetings. Build strong working relationships with

partner organisations. Leadership, Supervision and Workforce Lead and support Health Care Assistants in delivery of care
