About the Role
Registered Nurse (Hospice / Palliative Care) Job Purpose Plan and deliver patient-centred care, working closely with
patients and their families. Once trained, act as the shift coordinator, overseeing high-quality team nursing care and
coordinating wider team care that puts patients' at the heart of what we do. Act as the key worker for a designated
caseload, showing nursing leadership by providing care as part of a multidisciplinary team. Key Responsibilities
Clinical Intervention 1. Places quality at the centre of care by providing evidence-based, individualised, and
personalised support through holistic needs assessment, risk assessments, care planning, and evaluation of all care
