About the Role

Job Purpose To provide community nursing care to a wide variety of patients in residential homes across Nottingham City.

To work within the wider integrated care homes service providing holistic care to patients. This nursing care includes

acute and chronic wound management, medication administration, cathetercare, palliative care, long term conditions/

self-care. To provide holistic and individualised care to patients and carers as delegated by the Team Leader or deputy,

working autonomously and as part of a team. To regularly take responsibility for care and workload management in

theabsence of the Team Leader or deputy with named support from anotherteam. Dimensions To provide nursing care across a
