About the Role
Main Duties and Responsibilities Patient Care: Carry out holistic assessments, considering physical, psychological,
social, spiritual and ethical needs Support advance care planning, including ReSPECT discussions Plan, deliver, and
coordinate care in collaboration with the multi-professional team, ensuring patient preferences and dignity are upheld
Provide nursing, personal care, and symptom management, administering medications as prescribed and in line with
clinical guidance Provide symptom management support and recognise when needs escalate Educate and support patients and
families on disease progression, care options, and end-of-life care Ensure effective and sensitive communication with
