About the Role

We are looking for enthusiastic, skilled and motivated Rehabilitation Assistant to join our Maximising Independence

service across Wandsworth borough to deliver high quality, timely care support with an enablement focus to patients in

the community.

The Maximising Independence service provides rapid response social care services, in the community for patients who are

at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to

hospital due to an overriding social care need.
