About the Role

To work as a member of the Intermediate Care Team central Manchester, providing direct care for patients in the

community who have been referred to the service

The team are multi-professional and multi-agency and provide assessment and rehabilitation. Their aim is to maintain

people in their own homes, prevent re-admission to hospital or inappropriate admissions to residential or nursing homes,

by maximising function and independence. Care is delivered in a variety of settings including units and patients own

homes and their community.
