About the Role

JOB SUMMARY The post holder will: Under the direction of registered health care professionals/associate practitioner,

perform delegated routine interventions and care tasks, for which they have received training, seeking support as and

when necessary. As part of the neighbourhood rehabilitation team, support appointments with service users in their homes

and the community (including care homes), primarily lone working under direction or with other team members to provide

the highest standards of care and compassionate planned care to people that maximises their recovery and rehabilitation
