The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing
care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from
hospital or prevent admission to hospital. The post holder will undertake a range of delegated tasks, following a care
plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this
client group to provide the appropriate level of care.
