designed to support patients at home, prevent unnecessary hospital admissions, and deliver compassionate, timely care
through innovative virtual solutions.
As a Remote Monitoring Clinical Delivery Practitioner within Community Care, you’lloversee utilisation of remote
monitoring licenses and equipment across community services. You will support the development of future clinical
pathways and provide clinical advice and guidance to internal and external stakeholders including clinicians, care
