About the Role

The post holder will be responsible for the delivery of high-quality care to a defined patient group either withinthe

acute hospital or community-based setting, under the supervision of a registered professional. The postholder will

undertake the assessment, planning, organisation, delivery, and evaluation of evidence basedindividualised patient care
