About the Role

Please see attached job description Operational Work closely with GP/PCN Care Coordinators / GP Assistants to identify

patients who have been lost to follow up, and are not being served by the current diabetes service. This includes

patients who are of working age, LD, vulnerable, housebound, complex care requiring MDT approach, language barriers,

concurrent mental health problems etc. Perform phlebotomy Increase 8 Care processes data collection Present patients at

a diabetes MDT if required Write back/discharge to GP, unless updating EMIS directly Liaise with participating practices

to book patients for reviews Review patient goals Complete appropriate onward referrals e.g., to foot health, structured
