About the Role
About us
Transforming Care in Salford is an innovative project supporting families with Children with Learning Disabilities,
Autism and sometimes mental health issues to prevent them entering the Care system or requiring a Tier 4 hospital
admission.
Salford was one of two Pilots in GM developing the core service of offering Clinical Phycological support in addition to
that of a Learning Disability nurse to develop Positive Behaviour Support plans and later the Keyworker.
