To work under the direction of the Deputy Manager Registered services for Reablement, Domiciliary Care Services,
services on a locality level.
To support reablement and care support workers to promote wellbeing, early intervention, and prevention to deliver
outcomes focused care and support through strength-based personal plans.
To schedule a team of Reablement and Care Support Workers area, ensuring the smooth delivery of care, intervention
