About the Role

JOB DESCRIPTION

The Addictions Practitioner will work as a member of a multi-disciplinary team across the range of Health in Prison

addiction service Liaise with community addiction services. They will ensure that a responsive high quality service is

provided to service users, their families and carers. They will work within a matrix organisation as a member of the

multi-disciplinary Health in Prison addiction team promoting effective inter-disciplinary and inter-agency working

relationships and acting as a link between clients, professionals and other services to ensure continuity of care. They
