About the Role
JOB DESCRIPTION
The post holder will have responsibility for providing specialist nursing skill, knowledge and expertise in the care and
management to patients referred to the Home Treatment Team. The post holder will participate in the co-ordination of
gate keeping of acute care referrals from a wide range of services. The post holder will facilitate the provision of
expert mental health nursing intervention and support to service users and their families and will undertake and
participate in audit, research and development of the Home Treatment Team. The post holder will be an advocate to
