About the Role
The Senior Clinical Practitioner will utilize advanced skills to assess, diagnose, and manage patients with palliative care needs in various settings, including homes and hospitals. This role involves working collaboratively within a multi-disciplinary team to ensure high-quality patient care and advocate for patient wishes, ultimately aiming to prevent unnecessary hospital admissions and enhance care at home.
Responsibilities
- Rapidly assess patients with palliative care needs.
- Develop management plans based on patient wishes.
- Screen patients for unmet palliative care needs.
- Lead formal and informal teaching sessions.
- Support advance care planning and education.
