Senior Community Learning Disability Nurse (CPH)

About the Role

It is expected that the post holder: Applies evidence-based care to support physical well-being utilising Learning

Disability specific and mainstream health targets. Can assess and manage complex clinical issues in relation to service

users;Can advise and collaborate with carers; professionals; other agencies to around complex clinical interventions and

outcomes. Assesses, implements and critically evaluates intervention outcomes to maintainand improve the physical

well-being of service users. Involves otheragencies/services as appropriate to meet need. Liaises with

multi-disciplinary colleagues and other professionals / agencies toaddress issues that create barriers to social
