Senior Community Mental Health Nurse (CPN)

About the Role

As a Care Coordinator, you will hold a caseload of service users and have a number of responsibilities that include

assessment and coordination of care needs. Once needs are identified, you will be expected to plan and implement

evidence-based care to the service users and their carers. You will have the support and supervision of your Team Lead,

and as a Band 6 clinician, you will be expected to act as a supervisor and resource for junior or less experienced

staff. You will be able to draw on the interventions and expertise provided by your MDT colleagues, such as Support

Workers, Social Workers, Occupational Therapists, Psychologists, Employment Specialists, Family Therapists and Medical
