About the Role

The West Essex Specialist Dementia and Frailty Service (SDFS) is an assessment, diagnostic and support pathway for

people with dementia of any age, as well as people over the age of 75 who have complex mental health needs and an

associated age related frailty.

We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local

organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred

collaborative approach.
