About the Role

Are you a band 6 or an experienced band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated,

and dynamic Nurse with a passion for high quality care and service development.

You will be responsible for managing your own clinical caseload of community patients well as supporting the

HCA/rehabilitation assistants with training, supervision, and joint sessions.

The Rehabilitation And Frailty Team (RAFT) provides daily input for up to 21 days in the patient’s own home. The service

hours are 8am – 8pm with a care element added 7am – 10pm 365 days a year. You will be working occasional weekends and
