About the Role
JOB DESCRIPTION
Support the District Nurse in the delivery and co-ordination of community nursing services, making the best use of
available resources. Deputise for the District Nurse and support with case management as required. Work autonomously to
provide holistic person-centred care for those on a District Nursing caseload and provide professional advice within the
