About the Role
JOB DESCRIPTION
The Successful post holder will work within Disability Day Care Services in the Southern Health & Social Care Trust,
providing person centred nursing care and support to service users, their families and carers within Disability Day Care
settings. The post holder will be expected to work flexibly across day care settings trust wide as required to meet the
needs of the service. The post holder will work collaboratively as part of a multidisciplinary team, ensuring the
delivery of responsive care by assessing, planning, implementing and evaluating care to meet assessed need within
