Senior Health Care Assistant
About the Role
a. Support the practice nurse with health promotion programmes b. Support the practice nurse with the management of
Chronic Disease clinics c. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse
rate, calculating & recording average readings from SystmConnect & AccuRx, and making appointments as needed d. Review
patient triage submissions and making nursing / HCA appointments as needed, review incoming tasks to the nurses inbox
and book where appropriate, for example SystmConnect submissions e. Dip urine samples, record and share results as
appropriate, and send to lab f. Facilitate routine and 24-hour BP monitoring, advising patients accordingly g. Undertake
