About the Role

Our Community Teams provide compassionate, person-centred care to people living with progressive and life-limiting

illnesses across Stoke-on-Trent & North Staffordshire. Working alongside Clinical Nurse Specialists (CNS) to deliver

holistic, responsive, and person-centred care to patients in the community.

As a Senior Health Care Support Worker , you’ll play a key role in coordinating day-to-day patient contact within a

community geographical team. This role requires excellent communication skills, confidence working independently, and a

caring, organised approach to supporting patients, families and colleagues.
