About the Role

You will therefore need to demonstrate your abilities in the following areas: Proactively contribute to the delivery of

high-quality safe evidence-based care. Responding to SOS calls visiting patients in their own home to provide palliative

and end of life care. To lead on and co-ordinate assessment, planning and evaluation of individualised patient centred

care. Ensuring a holistic approach that is sensitive and responsive to the patients ever-changing needs. To be

responsible for the co-ordination and smooth running of a shift and visits ensuring it is well organised. As a member of
