About the Role

We are currently looking to recruit a full time Band 6 Mental Health Practitioner (Mental Health Nurse, Occupational

Therapist or Social Worker) to join our small and supportive Community Mental Health Team for older adults, embedded

within an exciting Integrated Care model. This is a great opportunity to join our friendly and dynamic multidisciplinary

team, with lots of support and CPD opportunities.

We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with

a range of Functional and Organic mental health illness. You will manage a caseload of service users, delivering a range
