About the Role

clinical management function of the CHC service, forming part of a senior duty roster to handle fast track applications,

review checklist referrals, manage complex cases and sign off assessment ratifications. The post holder will ensure that

all care plans are in place which are up to dayte and support the clients current care needs. They will also ensure

there is a regular review process in place for the care plans and where these have not been updated or are omitted from
