Senior Nurse Practitioner - Urgent Community Response

About the Role

You will use established pathways and processes to provide a wide range of nursing, therapy and social care assessments

to patients who may potentially be at risk of a hospital admission. The successful candidate will also work with other

clinicians including Geriatricians, Physiotherapists, Occupational therapists and social workers, whom are all

co-located within the team. We welcome applicants from a range of backgrounds including emergency nurses, primary care

and acute medicine as well as nurses from community backgrounds who have experience in working within Integrated

Services.
