About the Role
JOB DESCRIPTION
The AC@HT primary focus is on maintaining older people at home in the event of a crisis or unexpected deterioration in
health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and facilitate early
discharge. The Rehabilitation Health Care Assistant will carry out ongoing delegated nursing / therapy support to
patients who require ongoing support, using a patient centred approach. The duties to be carried out with each patient
will be clearly specified by the nurse / physiotherapist / occupational therapist and will reflect the assessed need of
