About the Role
The Integrated Case Management Team is seeking an experienced and motivatedBand 6 Senior Occupational Therapistto join
our multidisciplinary service delivering hospital-level care in patients’ homes. Working closely with community and
acute partners, the service supports patients referred from GPs, Rapid Response, A&E, and other services, carrying out
holistic home-based assessments and developing person-centred treatment plans in collaboration with consultants through
virtual ward rounds.
You will work within a supportive multidisciplinary team, including Consultants, GPs, Nurses, Clinical Pharmacists,
