About the Role

This is an exciting opportunity to work within the Enhanced Support Team alongside OT SLT and nursing colleagues. The
role requires the post holder to visit patients with a learning disability in their homes and day opportunities, working
with the wider team (Psychiatry Psychology and Assessment and Treatment Unit) to deliver care to prevent mental health
hospital admission and ensure patients can be supported in their home.
The post holder will be required to work alongside OT ,SLT and nursing colleagues in the Adult Learning Disability Team
