About the Role

The post holder will act as an experienced physiotherapist, helping to develop integrated health care teams in the

communitywith ina Multi-Disciplinary Team (MDT). The post holder will provide high quality physiotherapy interventions

to a range of patients from different community settingssuch as:frailty, rapid response / admission avoidance,

intermediate care, falls prevention through to preventative action and support for self-management oflong

termconditions. The caseload will include advanced assessment and treatment of patients, provision of individual and

group education and treatment, provision of assistive devices (e.g.orthotics) and equipment as well as liaising
