About the Role

An exciting opportunity has arisen for a senior Integrated care coordinator to join Rutland County Council’s RISE team

This role focuses on the development of the integrated Rutland neighbourhood team by promoting positive relationships

and partnerships with key agencies and professionals working with residents and those accessing primary care in Rutland.

The aim is to promote social inclusion and support people with long term health and social care needs by collaborating

with community organisations, as well as providing up-to-date information and advice to ensure that people can connect

to their community and access support for themselves
