About the Role

Promote people to live safely and confidently at home, working with the team, including: o Completing the Home pack that

includes a number of checklists and assessments, communicating any feedback to the team, individual and family, o

Providing risk assessments and feedback of the persons situation and ability, o Assisting the person to develop and work

on outcome focussed goals enabling them to relearn or develop new skills and abilities required for daily living. o As

appropriate to provide practical, personal and/or intimate care whilst maintaining high levels of hygiene and health and

safety. o Reviewing progress, providing feedback to the team, the person and their family to optimise independence and
