About the Role

An exciting opportunity has arisen for an nurse with specialist palliative care experience to work in our newly reopened

Day Services. A new model for the service is being delivered incorporating outpatient clinics and patient and carer

education sessions to encourage self-management. We are also keen to explore ways in which we can expand the service to

reach those patients and carers who would normally not engage with our services.

Meadow House provides a specialist palliative care service for the residents of the boroughs of Ealing and Hounslow. The

Specialist Palliative Service comprises of a 15 bedded in-patient unit, day services and a large multidisciplinary
